Hospital2Home will contact and welcome the patient, answer initial questions, and arrange the first delivery of their feed and/or equipment, at a convenient time and place. Working closely with our Abbott Nurse Advisors, Hospital2Home also ensures patients receive training, support, and their nutritional supplies when and when they need them.

Our video series welcoming patients to the Hospital2Home service provides an enhanced onboarding experience for adults and children. We now offer more support in more formats, offering even more patient choice.
PERSONALISED
interactions based on patient preferences help to create a compassionate and understanding service. Welcome Packs reflect changing customer needs and are tailored for adult and paediatric patients across differing healthcare systems.
PRACTICAL
patient-facing website empowers patients to manage their orders at their convenience. Following a major cloud-based platform upgrade, our telephone system includes a convenient Queue Call-Back feature for more accessible experience.
Our Hospital2Home teams are available to provide support for queries relating to loaned FreeGo feeding pumps.
We have emergency depots across the UK* and replacement pumps can be delivered within six hours if required.
If required by patients or carers, our Hospital2Home team will liaise with a patient's healthcare professional
and Abbott Nurse Advisor to provide further support.

TRAINING AND SUPPORT
Abbott Nurse Advisors can support patients by offering training on enteral feeding and equipment at their convenience. They can also be available to provide clinical advice and support to patients throughout their enteral feeding journey.
PATIENT ONBOARDING
The Hospital2Home team will contact patients and welcome/onboard them onto the service, answer any initial questions and arrange the first delivery of feeding equipment at a convenient time and place. They are also available to provide support throughout a patient's journey with us.
HOSPITAL2HOME WEBSITE
For ongoing support, the Hospital2Home.abbott website is available to Abbott Hospital2Home patients to place monthly orders, provide pump support and track/view orders.
FREEGO SUPPORT
The Hospital2Home team are available to provide support for any queries relating to the loaned FreeGo feeding pump. We have emergency depots across the UK* and replacement pumps can be delivered within six hours if required, night or day.
TEAM LIAISON
If required by patients or carers, the Hospital2Home team will liaise with the healthcare professional and the Abbott Nurse Advisor to provide further support.
