Abstract

Gastrointestinal (GI) intolerance is much more common than food allergy and has distinctly different causes and symptoms.1 GI intolerance happens when the digestive system cannot process some foods, causing an unpleasant physical reaction to them.1 Unlike a food allergy, it does not involve an immune system reaction - in fact, people with GI intolerance can usually tolerate small amounts of these foods without experiencing any uncomfortable symptoms.1

The symptoms of GI intolerance happen quite slowly (often many hours after eating the offending food), last for a long time and include bloating and stomach pain.1 Because the symptoms are very similar to those caused by other conditions (for example, inflammatory bowel disease or irritable bowel syndrome), GI intolerance can be difficult to accurately identify.1

She was admitted to hospital with chronic diarrhoea, and investigation showed evidence of ulceration of the ileosigmoid anastamosis and terminal ileum. During this admission she had numerous medical interventions such as steroid therapy, blood and ferritin transfusions, diuretics and a trial of Creon. She was fed via nasogastric (NG) feeding tube as she was only consuming half of her meals. Rupert is a 65 year old male with Ménière’s disease, chronic obstructive pulmonary disease (COPD), depression, angina and previous history of transient ischaemic attacks (TIAs). Rupert was admitted to hospital with gastroenteritis and poorly controlled angina. He develop loose stools (type 7 on the Bristol Stool Chart) which persisted for four weeks and Metronidazole was prescribed for suspected C.difficile. His loose watery stools persisted and after a flexible sigmoidoscopy and colonoscopy were performed, Rupert was diagnosed with non-infective gastroenteritis and colitis. Codeine phosphate, Citalopram, Omeprazole, Prednisolone were trialled during this time. Rupert was only managing to consume a quarter of his meals and was prescribed whole protein oral nutritional supplements (ONS).
