to reduce unnecessary hospital readmissions through early home support, ongoing monitoring, routine reviews to identify and manage issues, and troubleshooting advice.
DISCHARGE TRAINING SERVICE
The Abbott Nurse Advisor team works closely with NHS clinical teams and Abbott’s Hospital2Home service
to ensure that patients and their caregivers receive tailored education, information and support to manage their
enteral feeding plan and prepare them for their transition to home.
TRAINING & SUPPORT TO HCPS
Our Abbott Nurse Advisor team provides training and ongoing healthcare professional support, with 98% of participants rating the overall quality of the training and support provided as excellent or good (518 HCPs during the period January - December 2024).
SUPPORT POST DISCHARGE
The Abbott Nurse Advisor Service may reduce unnecessary hospital re-admissions by providing:
- Early home support post-discharge to confirm that patients and their caregivers are competent and confident to manage their enteral feeding at home; and that they have all of the supplies that they need.
- Ongoing monitoring and routine reviews to ensure that patients are supported at home and that any issues identified are managed appropriately.
- Troubleshooting advice and clinical support to ensure that patients are able to continue feeding
safely at home.
