Working with Hospital2Home, we facilitate safe and effective discharge from hospital, with ongoing clinical support in the community with direct access to a named Abbott Nurse Advisor for each patient. Through personalised training and tailored education our team empowers patients and caregivers to self-manage their home enteral feeding (HEF) requirements.

The Abbott Nurse Advisor team works closely with NHS clinical teams and Abbott’s Hospital2Home service to ensure that patients and their caregivers receive tailored education, information and support to manage their enteral feeding plan and prepare them for their transition to home.
Our Abbott Nurse Advisor team provides training and ongoing healthcare professional support, with 98% of participants rating the overall quality of the training and support provided as excellent or good (518 HCPs during the period January - December 2024).
