By using the Abbott Website, you confirm that:

• You are a healthcare professional registered in the United Kingdom;
• You are ordering an Abbott product sample for a patient to evaluate the product;
• You have not previously ordered an Abbott product sample for the patient for whom you are intending to submit an order;
• You accept these Online Terms of Use and agree to comply with them; and
• You will not bill, charge or sell to, or seek reimbursement from, any third party, including any insurer or managed care or government reimbursement programme, for such samples.

If you do not agree to these Online Terms of Use, you must not use the Abbott Website. We recommend that you print a copy of these Online Terms of Use for future reference.

These Online Terms of Use govern your access to the Abbott Website. These Online Terms of Use do not apply to Abbott websites that do not link to these Online Terms of Use, to residents of the United States or to third party websites to which the Abbott Website may link.
