Our Hospital2Home homecare service is made up of committed and caring professionals who offer a personalised service to support a patient's daily routine. 

Our Abbott Nurse Advisors work closely with our Hospital2Home advisors in order to ensure patients receive training, support and their nutritional supplies when and where they need them.

Abbott Nurse Advisors can support patients by offering training on enteral feeding and equipment at their convenience. They can also be available to provide clinical advice and support to patients throughout their enteral feeding journey.

Our Hospital2Home team support a patient's journey from hospital discharge, through to their return to home. They will contact patients and welcome them onto the service, answering initial questions and arrange the first delivery of their feed and/or equipment at a convenient time and place. They are also available to provide support throughout a patient's journey with us.

For ongoing support, the Hospital2Home.abbott website is a great option to both patients, carers and care homes who need to place monthly orders, need pump support, or would like to track or view their orders. The website facilitates self-care management and contains useful support information such as video guides on how to use the FreeGo pump.

The Hospital2Home team are available to provide 24/7 support for any queries relating to the loaned FreeGo feeding pump. We have emergency depots across the UK* and replacement pumps can be delivered within six hours if required, night or day.

If required by patients or carers, the Hospital2Home team will liaise with a patient's healthcare professional and the appropriate Abbott Nurse Advisor to provide further support. We also have dedicated leads for each different Trust or NHS Organisation that we serve.
