Assess key risk factors to validate whether the child identified in Step 1 is at risk of growth issues. Complete the questions below based on clinical assessment and history. Interpret findings alongside clinical judgement.
1. Does the child's growth deviate from expected growth?
Consider parental stature
2. Are there any environmental or social factors that may affect the child’s growth?
Financial stress, food insecurity, limited sleep routine, high screen time, low physical activity opportunities, caregiver mental health challenges, exposure to smoke
3. Does the child have any acute or chronic condition/s that affects growth?
Increased/reduced nutritional losses or requirements
4. Does the child show challenging eating behaviours?
Picky eating, overeating, food refusal or neophobia
5. Has the amount the child is eating or drinking changed?
No risk factors identified at this time. Schedule an appointment to reassess weight and height in 3 months.
One or more risk factors were identified.
Children identified as at risk of growth issues may benefit from early, targeted intervention. Consider evidence‑based strategies such as:
1) Healthy, balanced diet guidance (food and fluid intake)
2) Healthy feeding behaviour strategies (eg. meal frequency, feeding skills)
3) Age‑appropriate physical activity
4) Multi‑nutrient oral nutritional supplement if intake is inadequate
Continue intervention for 3 months and then schedule a STEP 4: REVIEW appointment.
If this is a review and there is:
NO IMPROVEMENT:
REFERRAL to appropriate medical specialists or allied health providers based on needs.
IMPROVEMENT:
Continue with intervention and REVIEW IN 6 MONTHS.
