Abstract:

Following recent changes in sending Home Enteral Feed (HEF) prescriptions to a HEF supplier, we have put together some simple instructions on setting up one-off nominations.

 

 

Introduction

The introduction of one-off nominations eliminates the need for practices to issue FP10s, Phase 4 tokens, or modify patient pharmacy nominations for nutritional items. Instead, Abbott Hospital2Home (H2H Pharmacy Limited) is now available as an option under the one-off nominations.

 
What is a one off nomination?

A one-off nomination is used to temporarily change a patient's nominated pharmacy for a single prescription. This doesn't impact the patient's regular nominations, and future prescriptions will still go to their usual pharmacy. However, if the patient's regular pharmacy nomination is modified before the prescription is downloaded, the one-off nominated prescription will be redirected accordingly.

For all SystmOne users and some EMIS Web users, one-off nominations are now accessible. If your practice doesn't have this feature yet, you can contact EMIS directly to enable it.

Additionally, there is the option to set up H2H as the patient's primary pharmacy nomination, which is useful when patients only need HEF products and don't require any other medications from their regular pharmacy.

The process outlined is based on the most accurate information provided to Abbott in February 2023. These steps are subject to change, in line with prescribing system updates.

 
SystmOne Guide for setting up one-off nominations

1. Issue item and select the 'One-off-nomination' box.

2. To find Abbott Hospital2Home (H2H Pharmacy Limited), you must now enter their ‘dummy postcode’ (ZE1 0AA) as the supplier’s actual postcode will no longer find them. Make sure the 'Other' button is selected.

3. Then one-off nomination box will show the chosen pharmacy's details:

1. Issue the item from the repeat or acute list following the practice’s usual process. Once on the issue screen, click EPS nominations at the top right of the pop up box.

Note: If your practice is not enabled for one-off nomination then please contact EMIS to see if it may be enabled on your system.

 

2. The ‘Manage EPS Nominations’ box will pop up. There is an option for a one-off pharmacy nomination. Click on find.

3. The ‘One-Off Pharmacy nomination search’ box will pop up. Search for the supplier by postcode. Use the ‘dummy’ postcode (ZE1 0AA) as the supplier’s actual postcode will not work. Choose the correct supplier and click OK.

4. The chosen one-off nomination will show on the ‘Manage EPS Nominations’ screen. Press OK and then continue to issue the prescription following the usual practice processes.

The benefits of Abbott Hospital2Home deliveries

The H2H service will only request prescriptions and supply feed on behalf of patients that have chosen for them to do so. Benefits of the H2H service delivering enteral feed supply on a monthly basis include:

  • Patients receive monthly reminders and a delivery is never made without a stock check being completed
  • Feed can be bulky and heavy, we deliver to the patient’s door at a time and date convenient for them
  • The H2H Coordinator will confirm the preferred flavours (where applicable) and bottle size with the patient, ensuring they don’t receive a feed they cannot use
  • If a Dietitian recommends an amendment to the patient’s enteral feed and this change needs to be made as soon as possible, the required feed can be delivered the next working day provided a prescription has been issued and the amendment has been received by 12pm midday
  • Our pharmacists check the prescription against the latest regimen to avoid incorrect feed being sent. Local community pharmacies do not have access to the latest regimen and are at risk of dispensing against the original regimen without knowing it has been changed
