Abbott Hospital2Home provides committed support to the transition and provision of ongoing patient support when care is moved from hospital back into the community. The service connects seamlessly to provide patient and HCP reassurance during this transition and continuity of care.
Our Hospital2Home homecare service is made up of committed and caring professionals who offer a personalised service to support a patient's daily routine.
Our Abbott Nurse Advisors work closely with our Hospital2Home advisors in order to ensure patients receive training, support and their nutritional supplies when and where they need them.

Our eRegistration service is just one of our digital solutions that we provide to the NHS. It is available to all Abbott contract areas and supports NHS cost-saving via electronic patient management.
Our unique purpose-built operations facility at Sittingbourne combines the meticulous distribution of patient prescriptions and sample orders with device servicing and maintenance
Our contract managers are a dedicated point of contact. They are a highly responsive team who provide consistent monitoring to maximise contract performance and satisfaction, ensuring swift issue resolution, collaboration between NHS organisations and Abbott and sharing best practice between contracts to help with service development.

Vital 1.5kcal
Vital 1.5kcal is suitable for people with disease-related malnutrition and malabsorption or for those who experience symptoms of poor feed tolerance.
Jevity Advance
Jevity Advance is a 1.5kcal/ml, nutritionally complete† tube feed with FOS (fructo-oligosaccharides), and a unique blend of high protein, additional vitamin D and CaHMB (calcium-β-hydroxy-β-methylbutyrate). Jevity Advance is available in 500 ml Ready to Hang bottles, which attach directly to Abbott giving sets.
TwoCal
TwoCal / TwoCal Bolus is a 2.0 kcal/ml tube feed for people who are catabolic or fluid restricted who have, or are at risk of developing, disease-related malnutrition. TwoCal / TwoCal Bolus contains FOS (fructo-oligosaccharides), which have an important role in the diet by helping to support gastrointestinal function.1 It is available in a 1000 ml Ready to Hang bottle and a 200ml bottle for bolus feeding which is known specifically as TwoCal Bolus. Both presentation sizes attach directly to Abbott giving sets.
